How to Apply for VA Medical & Prescription Benefits
To apply just read the information on this page and fill out the 10-10 EZ form.

1. Obtain a copy of your DD214. (Note: if you do not have a copy of your discharge or a DD214 please call my office at: 734-8340 and I will get a copy for you.) |

2. Call the Veterans Service Officer for your town/city. If unavailable, contact Bill Bradley at 413-743-8340 to schedule an appointment. |

3. When you come in for your appointment bring your completed 10-10 E Z Form and Discharge / DD214,
it will be checked over, corrections made where needed, copies made for your file in the Town Hall and copies for your personal at home files.
(Note: If for physical or other reasons you are unable to fill out the 10-10 EZ Form don't worry. VSO can it out for you.)

Your 10-10 EZ application will be sent to the VA. Two weeks after your
application reaches the VA you will need to call the VA Clinic in Pittsfield at: (413) 443-4857. The VA Clinic
is located at 73 Eagle Street. Eagle Street is a one-way street that runs from North Street to First Street,
and is the nearest VA Clinic to the Northern Berkshire area.

When you call the VA Clinic in Pittsfield, ask them to check to see if you are in the
computer. If you are listed, ask to make an appointment to get your personalized VA card made. If you need
to see a doctor, ask for an appointment. (Note: If you are being treated by your personal physician/s
please obtain a letter/s with your diagnosis and prescription needs to bring to your VA appointment.
The VA doctor will re-evaluate your diagnosis and issue new prescriptions through the VA's Prescription
By Mail Plan.

If you need transportation to the clinic, call one of the Free Van Services. These FREE
VAN SERVICES are available on a first come, first served basis, so arrange your transportation as soon as
possible. Many of the Van Services listed are restricted and can only transport you to certain locations.
Please check listed information before calling. Because of funding problems some of these services are out
of service from time to time.

other State or VA benefits, please call your city/town's VSO. Or you may contact Bill Bradley at 413-743-8340,
located on the 3rd floor of the Adams Town Hall. (Elevator Available) Hours are 8:30 AM to 4:00 PM, Wednesday,
Thursday, and 8:30 AM to 3:30 on Friday. (NOTE: To better serve you it is advised that you make an appointment so you
will not wait long.)
